Care coordination promotes cooperation and information-sharing between multiple providers and specialists serving a patient, reducing fragmentation throughout care. With a registered nurse and licensed social worker working to provide seamless patient care, here’s what can be offered by YMC Care Coordination:

- Centralized assistance- especially when they have multiple issues/providers
- Follow up routinely to get barriers and issues resolved
- Provide referrals for community resources
- Assist with applications for Medicare B Savings/Extra Help/Prescription Assistance when the patient has difficulty affording medications or insurance premiums
- Coordination with VA and outside providers and facilities to provide seamless patient care
- Referrals for in-home needs when home safety is a concern (cleaning, med management, DME equipment, home health, meals, shopping, transportation, lifeline, etc.)
- Offer knowledge regarding advanced directives, Medicare screenings, and general Medicare questions
